It is an emotional, biological, and behavioral condition that requires patience, consistency, and a professional therapeutic approach. In the article <em data-start=\"517\" data-end=\"624\">\u201cAlcohol Dependence \u2013 Mechanisms, Psychology, Examples and Professional Therapeutic Approach in Therapy,\u201d<\/em> I already explained how the addicted brain works, why a person cannot \u201cjust stop,\u201d and which neurobiological mechanisms drive the compulsive use.<\/p>\n<p data-start=\"772\" data-end=\"1110\">Here, we continue logically forward \u2014 <strong data-start=\"810\" data-end=\"873\">how therapy with an alcohol-dependent person actually works<\/strong>, and how work is done both in the therapeutic process and within the family system.<br data-start=\"957\" data-end=\"960\" \/>Alcohol dependence is a topic that always involves two sides: the dependent person and the system around them \u2014 partner, children, parents, employers.<\/p>\n<p data-start=\"1112\" data-end=\"1194\">Many articles confuse these roles.<br data-start=\"1146\" data-end=\"1149\" \/><strong data-start=\"1149\" data-end=\"1192\">Working with addiction has three sides:<\/strong><\/p>\n<ol data-start=\"1195\" data-end=\"1268\">\n<li data-start=\"1195\" data-end=\"1213\">\n<p data-start=\"1198\" data-end=\"1213\">the therapist<\/p>\n<\/li>\n<li data-start=\"1214\" data-end=\"1243\">\n<p data-start=\"1217\" data-end=\"1243\">the close family members<\/p>\n<\/li>\n<li data-start=\"1244\" data-end=\"1268\">\n<p data-start=\"1247\" data-end=\"1268\">the addicted person<\/p>\n<\/li>\n<\/ol>\n<p data-start=\"1270\" data-end=\"1380\">The therapist works in one way.<br data-start=\"1301\" data-end=\"1304\" \/>The family works in another.<br data-start=\"1332\" data-end=\"1335\" \/>And the addicted person works in a third way.<\/p>\n<p data-start=\"1382\" data-end=\"1504\">The first two sides are supportive.<br data-start=\"1417\" data-end=\"1420\" \/>But <em data-start=\"1424\" data-end=\"1437\">real change<\/em> comes only from the behavior and decisions of the addicted person.<\/p>\n<h1 data-start=\"1511\" data-end=\"1542\"><strong data-start=\"1513\" data-end=\"1540\">HOW THE THERAPIST WORKS<\/strong><\/h1>\n<p data-start=\"1543\" data-end=\"1591\">with a person struggling with alcohol dependence<\/p>\n<h2 data-start=\"1598\" data-end=\"1662\"><strong data-start=\"1601\" data-end=\"1662\">1. The first session \u2014 the goal is safety, not accusation<\/strong><\/h2>\n<p data-start=\"1664\" data-end=\"1919\">The first session never starts with interrogation. It is not: <em data-start=\"1728\" data-end=\"1804\">\u201cHow much do you drink?\u201d, \u201cWhy do you drink?\u201d, \u201cDon&#8217;t you have willpower?\u201d<\/em><br data-start=\"1804\" data-end=\"1807\" \/>In my office, I create a sense of safety \u2014 the exact opposite of what the addicted person usually feels at home.\u00a0I speak calmly, and what I say is: <strong data-start=\"1958\" data-end=\"2060\">\u201cI understand that this is hard for you. Let\u2019s see together how we can help you feel more stable.\u201d<\/strong><\/p>\n<p data-start=\"2062\" data-end=\"2115\">During this session, the therapist does three things:<\/p>\n<h3 data-start=\"2117\" data-end=\"2178\">&#x2714; <strong data-start=\"2123\" data-end=\"2176\">1) Assesses the amount of use \u2014 without criticism<\/strong><\/h3>\n<p data-start=\"2179\" data-end=\"2214\">Not to judge, but to evaluate risk.<\/p>\n<h3 data-start=\"2216\" data-end=\"2279\">&#x2714; <strong data-start=\"2222\" data-end=\"2277\">2) Assesses whether withdrawal symptoms are present<\/strong><\/h3>\n<p data-start=\"2280\" data-end=\"2394\">If the person is trembling, sweating, unable to sleep \u2014 this can be dangerous and may require medical supervision.<\/p>\n<h3 data-start=\"2396\" data-end=\"2435\">&#x2714; <strong data-start=\"2402\" data-end=\"2433\">3) Calms the nervous system<\/strong><\/h3>\n<p data-start=\"2436\" data-end=\"2499\">Real therapeutic work begins only after the body is stabilized.<\/p>\n<p data-start=\"2501\" data-end=\"2648\"><strong data-start=\"2501\" data-end=\"2527\">Why is this important?<\/strong><br data-start=\"2527\" data-end=\"2530\" \/>The dependent person lives in constant anxiety \u2014 if they feel attacked in therapy, their brain immediately shuts down.<\/p>\n<h2 data-start=\"2655\" data-end=\"2731\"><strong data-start=\"2658\" data-end=\"2731\">2. Working on motivation \u2014 the person must want change for themselves<\/strong><\/h2>\n<p data-start=\"2733\" data-end=\"2862\">The therapist does not persuade with: <em data-start=\"2771\" data-end=\"2789\">\u201cStop drinking.\u201d <\/em>Instead, the therapist asks questions that help the person understand:<\/p>\n<p data-start=\"2864\" data-end=\"3134\">\u2726 What does alcohol give them? (calm, escape, silence in the mind)<br data-start=\"2930\" data-end=\"2933\" \/>\u2726 What does it take away? (family, health, trust)<br data-start=\"2982\" data-end=\"2985\" \/>\u2726 Which part of them wants sobriety? (the part tired of suffering)<br data-start=\"3051\" data-end=\"3054\" \/>\u2726 Which part is afraid? (the part that doesn\u2019t know how to live without alcohol)<\/p>\n<h2 data-start=\"3141\" data-end=\"3217\"><strong data-start=\"3144\" data-end=\"3217\">3. Trigger mapping \u2014 discovering the moments when the person \u201cbreaks\u201d<\/strong><\/h2>\n<p data-start=\"3219\" data-end=\"3284\">The therapist and the client sit together and create a clear map:<\/p>\n<p data-start=\"3286\" data-end=\"3455\">\u2022 At what moments does the urge to drink appear?<br data-start=\"3334\" data-end=\"3337\" \/>\u2022 Which emotions trigger the need?<br data-start=\"3371\" data-end=\"3374\" \/>\u2022 What automatic thoughts arise?<br data-start=\"3406\" data-end=\"3409\" \/>\u2022 Which people or places activate the craving?<\/p>\n<p data-start=\"3457\" data-end=\"3479\">These are things like:<\/p>\n<p data-start=\"3481\" data-end=\"3617\">\u2013 loneliness in the evening<br data-start=\"3508\" data-end=\"3511\" \/>\u2013 arguments at home<br data-start=\"3530\" data-end=\"3533\" \/>\u2013 meeting a specific friend<br data-start=\"3560\" data-end=\"3563\" \/>\u2013 anticipation of failure<br data-start=\"3588\" data-end=\"3591\" \/>\u2013 celebrations<br data-start=\"3605\" data-end=\"3608\" \/>\u2013 shame<\/p>\n<p data-start=\"3619\" data-end=\"3735\"><strong data-start=\"3619\" data-end=\"3631\">Example: <\/strong>\u201cWhen I hear someone raise their voice, my stomach tightens and I want to drink to numb the tension.\u201d This is the key: the person begins to understand themselves.<\/p>\n<h2 data-start=\"3804\" data-end=\"3869\"><strong data-start=\"3807\" data-end=\"3869\">4. Emotional regulation \u2014 new reactions instead of alcohol<\/strong><\/h2>\n<p data-start=\"3871\" data-end=\"3973\">Alcohol is not the problem. Alcohol is the <em data-start=\"3916\" data-end=\"3924\">method<\/em> a person uses to cope with overwhelming tension. In therapy, the person learns:<\/p>\n<p data-start=\"4007\" data-end=\"4199\">\u2022 how to calm their breathing<br data-start=\"4036\" data-end=\"4039\" \/>\u2022 how to recognize when they are close to the edge<br data-start=\"4089\" data-end=\"4092\" \/>\u2022 how to \u2018freeze the moment\u2019 before reaching for a drink<br data-start=\"4148\" data-end=\"4151\" \/>\u2022 how to wait 2 minutes, 5 minutes, 10 minutes<\/p>\n<p data-start=\"4201\" data-end=\"4295\">This is practical, body-based work.<br data-start=\"4236\" data-end=\"4239\" \/>It creates habits in the <strong data-start=\"4264\" data-end=\"4272\">body<\/strong>, not only in the mind.<\/p>\n<h2 data-start=\"4302\" data-end=\"4336\"><strong data-start=\"4305\" data-end=\"4336\">5. Breaking cognitive traps<\/strong><\/h2>\n<p data-start=\"4338\" data-end=\"4423\">Alcohol lives in the mind long before it reaches the hand. Common thoughts include:<\/p>\n<p data-start=\"4425\" data-end=\"4546\">\u2022 \u201cJust one.\u201d<br data-start=\"4438\" data-end=\"4441\" \/>\u2022 \u201cI did well today \u2014 I deserve it.\u201d<br data-start=\"4477\" data-end=\"4480\" \/>\u2022 \u201cI\u2019m irritated; better to drink.\u201d<br data-start=\"4515\" data-end=\"4518\" \/>\u2022 \u201cI can\u2019t live without it.\u201d<\/p>\n<p data-start=\"4548\" data-end=\"4650\">In therapy, the person learns to observe these thoughts like passing clouds \u2014 appearing, but not true.<\/p>\n<p data-start=\"4652\" data-end=\"4749\">&#x2714; \u201cLast time, \u2018one\u2019 became five.\u201d<br data-start=\"4685\" data-end=\"4688\" \/>&#x2714; \u201cI deserve care, not self-destruction.\u201d<br data-start=\"4729\" data-end=\"4732\" \/>&#x2714; \u201cI can wait.\u201d<\/p>\n<p data-start=\"4751\" data-end=\"4965\">At this stage, the client begins to recognize the <strong data-start=\"4801\" data-end=\"4818\">thought traps<\/strong> \u2014 those seemingly logical sentences that actually lead directly to drinking. The therapist helps replace them with thoughts that support sobriety.<\/p>\n<h2 data-start=\"4972\" data-end=\"5041\"><strong data-start=\"4975\" data-end=\"5041\">6. Working with the family \u2014 stepping out of destructive roles<\/strong><\/h2>\n<p data-start=\"5043\" data-end=\"5085\">The therapist helps the family understand:<\/p>\n<p data-start=\"5087\" data-end=\"5272\">\u2022 that rescuing harms<br data-start=\"5108\" data-end=\"5111\" \/>\u2022 that controlling harms<br data-start=\"5135\" data-end=\"5138\" \/>\u2022 that emotional distance harms<br data-start=\"5169\" data-end=\"5172\" \/>\u2022 that the addicted person is neither an enemy nor a child<br data-start=\"5230\" data-end=\"5233\" \/>\u2022 that everyone is part of the system<\/p>\n<p data-start=\"5274\" data-end=\"5309\">We work around one clear principle:<\/p>\n<p data-start=\"5311\" data-end=\"5431\">&#x2728; <strong data-start=\"5313\" data-end=\"5364\">\u201cI love you, but I will not cover anything up.\u201d<\/strong><br data-start=\"5364\" data-end=\"5367\" \/>&#x2728; <strong data-start=\"5369\" data-end=\"5431\">\u201cI support you, but you are responsible for your choices.\u201d<\/strong><\/p>\n<h2 data-start=\"5438\" data-end=\"5488\"><strong data-start=\"5441\" data-end=\"5488\">7. Relapse plan \u2014 because no one is a robot<\/strong><\/h2>\n<p data-start=\"5490\" data-end=\"5621\">Therapy prepares the addicted person for moments when they may slip \u2014 not because failure is expected, but because it is realistic.<\/p>\n<p data-start=\"5623\" data-end=\"5641\">The plan includes:<\/p>\n<p data-start=\"5643\" data-end=\"5789\">\u2022 early warning signs (irritability, restlessness)<br data-start=\"5693\" data-end=\"5696\" \/>\u2022 what to do in the first minute<br data-start=\"5728\" data-end=\"5731\" \/>\u2022 who to call<br data-start=\"5744\" data-end=\"5747\" \/>\u2022 how to avoid the \u201call-or-nothing\u201d trap<\/p>\n<p data-start=\"5791\" data-end=\"5833\">This gives the person peace and structure.<\/p>\n<h2 data-start=\"5840\" data-end=\"5873\"><strong data-start=\"5843\" data-end=\"5873\">8. Building a new identity<\/strong><\/h2>\n<p data-start=\"5875\" data-end=\"5936\">The final stage is not \u201cI don\u2019t drink.\u201d<br data-start=\"5914\" data-end=\"5917\" \/>The final stage is:<\/p>\n<p data-start=\"5938\" data-end=\"6044\">\u201cI am not a person who runs away.\u201d<br data-start=\"5972\" data-end=\"5975\" \/>\u201cI have a purpose.\u201d<br data-start=\"5994\" data-end=\"5997\" \/>\u201cI take care of myself.\u201d<br data-start=\"6021\" data-end=\"6024\" \/>\u201cI have boundaries.\u201d<\/p>\n<p data-start=\"6046\" data-end=\"6138\">The therapist helps build the new identity \u2014 not as a punished person, but as a renewed one.<\/p>\n<h1 data-start=\"180\" data-end=\"213\"><strong data-start=\"187\" data-end=\"211\">HOW THE FAMILY WORKS<\/strong><\/h1>\n<p><img loading=\"lazy\" decoding=\"async\" class=\"alignnone size-medium wp-image-1711\" src=\"https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-300x200.jpeg\" alt=\"\" width=\"300\" height=\"200\" srcset=\"https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-300x200.jpeg 300w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-1030x688.jpeg 1030w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-768x513.jpeg 768w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-1536x1025.jpeg 1536w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-2048x1367.jpeg 2048w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-1500x1001.jpeg 1500w, https:\/\/integritherapy.com\/wp-content\/uploads\/2025\/12\/help_alcoholic_family_member-705x471.jpeg 705w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/p>\n<p data-start=\"214\" data-end=\"295\"><em data-start=\"214\" data-end=\"295\">(this is an entirely different role \u2014 and it also requires therapeutic support)<\/em><\/p>\n<p data-start=\"297\" data-end=\"428\">The home can never be a therapeutic office. Family members <strong data-start=\"358\" data-end=\"368\">cannot<\/strong> \u2014 and should not \u2014 be the therapist of the addicted person.<\/p>\n<p data-start=\"430\" data-end=\"473\">But there is something even more important: <strong data-start=\"475\" data-end=\"516\">Very often the family is codependent.\u00a0 <\/strong>Meaning: they have lived for so long in tension, fear, guilt, and control that their reactions have become automatic and destructive.<\/p>\n<p data-start=\"653\" data-end=\"869\"><a href=\"https:\/\/integritherapy.com\/en\/2025\/12\/03\/codependencia-que-es-y-como-se-sana-guia-completa-de-petya-bankova\/\">Codependency<\/a> is a wound of its own \u2014 one that needs therapy. Not because the family is \u201cguilty,\u201d but because they are exhausted, hurt, and confused. Their patterns often maintain the addiction without realizing it.<\/p>\n<h1 data-start=\"876\" data-end=\"911\"><strong data-start=\"878\" data-end=\"909\">1. Support without rescuing<\/strong><\/h1>\n<p data-start=\"912\" data-end=\"968\">(<em data-start=\"913\" data-end=\"967\">rescuing is one of the main symptoms of codependency<\/em>)<\/p>\n<p data-start=\"970\" data-end=\"1017\">Family members often become \u201crescuers.\u201d They:<\/p>\n<p data-start=\"1019\" data-end=\"1129\">\u2022 cover consequences<br data-start=\"1039\" data-end=\"1042\" \/>\u2022 justify behavior<br data-start=\"1060\" data-end=\"1063\" \/>\u2022 stay silent<br data-start=\"1076\" data-end=\"1079\" \/>\u2022 lie<br data-start=\"1084\" data-end=\"1087\" \/>\u2022 pay debts<br data-start=\"1098\" data-end=\"1101\" \/>\u2022 hide embarrassing scenes<\/p>\n<p data-start=\"1131\" data-end=\"1219\">This is an attempt to protect the family, but in reality, it <strong data-start=\"1192\" data-end=\"1218\">prolongs the addiction<\/strong>. Healthy statements sound like:<\/p>\n<p data-start=\"1253\" data-end=\"1389\">&#x2714; \u201cI believe you can handle this.\u201d<br data-start=\"1287\" data-end=\"1290\" \/>&#x2714; \u201cI am ready to help if you want treatment.\u201d<br data-start=\"1335\" data-end=\"1338\" \/>&#x2714; \u201cI\u2019m here, but I can\u2019t do things instead of you.\u201d<\/p>\n<p data-start=\"1391\" data-end=\"1411\">Unhealthy behaviors:<\/p>\n<p data-start=\"1413\" data-end=\"1569\">&#x274c; covering up drinking<br data-start=\"1435\" data-end=\"1438\" \/>&#x274c; lying to employers or relatives<br data-start=\"1471\" data-end=\"1474\" \/>&#x274c; paying debts caused by alcohol<br data-start=\"1506\" data-end=\"1509\" \/>&#x274c; excusing drunken behavior<br data-start=\"1536\" data-end=\"1539\" \/>&#x274c; taking over responsibility<\/p>\n<p data-start=\"1571\" data-end=\"1643\">This is the essence of <strong data-start=\"1594\" data-end=\"1608\">tough love<\/strong> \u2014 warmth combined with boundaries.<\/p>\n<h1 data-start=\"1650\" data-end=\"1701\"><strong data-start=\"1652\" data-end=\"1699\">2. Conversations must be calm, not dramatic<\/strong><\/h1>\n<p data-start=\"1702\" data-end=\"1767\">(<em data-start=\"1703\" data-end=\"1766\">codependent people often swing between silence and explosions<\/em>)<\/p>\n<p data-start=\"1769\" data-end=\"1834\">In families affected by addiction, conversations often turn into:<\/p>\n<p data-start=\"1836\" data-end=\"1931\">\u2022 yelling<br data-start=\"1845\" data-end=\"1848\" \/>\u2022 accusations<br data-start=\"1861\" data-end=\"1864\" \/>\u2022 tears<br data-start=\"1871\" data-end=\"1874\" \/>\u2022 punishments<br data-start=\"1887\" data-end=\"1890\" \/>\u2022 threats<br data-start=\"1899\" data-end=\"1902\" \/>\u2022 silence from pain or fear<\/p>\n<p data-start=\"1933\" data-end=\"2019\">This creates a climate of <strong data-start=\"1959\" data-end=\"2019\">shame \u2014 and shame is the strongest trigger for drinking.<\/strong><\/p>\n<p data-start=\"2021\" data-end=\"2037\">Harmful phrases:<\/p>\n<p data-start=\"2039\" data-end=\"2118\">\u2718 \u201cStop drinking already!\u201d<br data-start=\"2065\" data-end=\"2068\" \/>\u2718 \u201cYou ruined our life!\u201d<br data-start=\"2092\" data-end=\"2095\" \/>\u2718 \u201cI\u2019m ashamed of you!\u201d<\/p>\n<p data-start=\"2120\" data-end=\"2166\">These deepen the pain and reinforce the cycle.<\/p>\n<p data-start=\"2168\" data-end=\"2189\">Helpful alternatives:<\/p>\n<p data-start=\"2191\" data-end=\"2331\">&#x2714; \u201cI see that you are struggling.\u201d<br data-start=\"2225\" data-end=\"2228\" \/>&#x2714; \u201cI will talk with you when you\u2019re sober.\u201d<br data-start=\"2271\" data-end=\"2274\" \/>&#x2714; \u201cI won\u2019t attack you, but I won\u2019t hide anything either.\u201d<\/p>\n<p data-start=\"2333\" data-end=\"2384\">This tone calms the system instead of exploding it.<\/p>\n<h1 data-start=\"2391\" data-end=\"2457\"><strong data-start=\"2393\" data-end=\"2455\">3. Family members must protect themselves and the children<\/strong><\/h1>\n<p data-start=\"2458\" data-end=\"2542\">(<em data-start=\"2459\" data-end=\"2541\">this too is therapy \u2014 because codependent people often cannot protect themselves<\/em>)<\/p>\n<p data-start=\"2544\" data-end=\"2634\">Living with a dependent person often leads to emotional illness. Family members develop:<\/p>\n<p data-start=\"2636\" data-end=\"2808\">\u2022 anxiety<br data-start=\"2645\" data-end=\"2648\" \/>\u2022 depression<br data-start=\"2660\" data-end=\"2663\" \/>\u2022 constant tension<br data-start=\"2681\" data-end=\"2684\" \/>\u2022 fear of outbursts<br data-start=\"2703\" data-end=\"2706\" \/>\u2022 feeling they must \u201cwalk on eggshells\u201d<br data-start=\"2745\" data-end=\"2748\" \/>\u2022 disrupted sleep<br data-start=\"2765\" data-end=\"2768\" \/>\u2022 guilt<br data-start=\"2775\" data-end=\"2778\" \/>\u2022 broken personal boundaries<\/p>\n<p data-start=\"2810\" data-end=\"2872\">Therefore, the family must protect themselves, which includes:<\/p>\n<p data-start=\"2874\" data-end=\"3072\">\u2022 financial safety<br data-start=\"2892\" data-end=\"2895\" \/>\u2022 emotional distance when needed<br data-start=\"2927\" data-end=\"2930\" \/>\u2022 clear boundaries: \u201cThis is not allowed in our home.\u201d<br data-start=\"2984\" data-end=\"2987\" \/>\u2022 self-care (sleep, therapy, support)<br data-start=\"3024\" data-end=\"3027\" \/>\u2022 protecting children from traumatic scenes<\/p>\n<p data-start=\"3074\" data-end=\"3111\">Family members have the right to say:<\/p>\n<p data-start=\"3113\" data-end=\"3246\"><strong data-start=\"3113\" data-end=\"3167\">\u201cI love you, but I cannot save your life for you.\u201d<\/strong><br data-start=\"3167\" data-end=\"3170\" \/><strong data-start=\"3170\" data-end=\"3246\">\u201cI can support you, but I cannot carry the consequences instead of you.\u201d<\/strong><\/p>\n<p data-start=\"3248\" data-end=\"3286\">This is an extremely healthy position.<\/p>\n<h1 data-start=\"3293\" data-end=\"3368\"><strong data-start=\"3295\" data-end=\"3368\">4.
